Terms & Conditions – June 2024
This summary will provide you with information about how I will work with you and/or your child. It will also explain my basic terms and conditions. I charge clients by the hour for face to face time. Included in this cost is the preparation time before each session, my travelling time and costs, record keeping and general administration required to ensure that your child’s therapy sessions are professional, individualised and fun!

Therapy/Treatment Session
I set aside an hour for each visit (therapy for 45 mins plus feedback/homework discussions before and after).
• Additional charges will apply for any report writing required.
• There is no charge for mileage or travel time as long as this does not exceed 15 miles in total or 30 minutes travel time in total from my base in Amersham. If mileage is exceeded an additional 0.45p/mile after the initial 15 miles will be added to your invoice.
• Missed appointments will be charged at the full rate unless 24 hours notice is given. If your child is seen in school and an appointment is missed due to school activities or absence, the full rate will be charged unless 24 hours notice is given.
• Fees should be paid at the end of each session by either cash or bank transfer, unless therapy is ongoing, in which case you will receive a monthly summary of therapy dates and amount owed via email, at which time you can make a bank transfer or pay cash at the next session. I ask that accounts are settled within 7 days from date of issue, late payment is subject to a £25 admin charge.
• If you have health insurance, payment of fees is your responsibility and a receipt will be issued once Family Physio has received payment, for you to submit a claim.

I would like to reassure you of my compliance with the Data Protection Act and current Health and Safety requirements. I also have a current DBS check. As an Independent Chartered Physiotherapist, I am registered with The Health Care Professionals Council (HCPC), The Chartered Society of Physiotherapy (CSP) and the Association of Paediatric Chartered Physiotherapists (APCP).

I regularly attend workshops, meetings, conventions and training days to ensure my clinical practices are up to date and in line with the latest research. I also regularly update my toys and resources so that your child has access to a wide variety of exciting activities, innovative resources and motivating games during our sessions.

My therapy is playful, relaxed and happy. If your child is not looking forward to our sessions, then please let me know. It is very important that their sessions with me are positive. If you have any questions about any of the above, please don’t hesitate to contact me.
